1. Approval of a General Liability Settlement Agreement between Ms. Karen Hartmann and Clay County Board of County Commissioners in the amount of $200,000.00. This is a lifetime settlement including attorney's fees and all present and future costs associated with Ms. Hartmann's injuries. The cost to the County will be the $100,000.00 insurance deductible. Additional approval to transfer $100,000.00 from General Fund Reserves for Contingency (001-9900-599100) to OCC-Insurance Deductible (001-0105-549103) to cover the settlement is requested. Approval of State Financial Assistance Recipient Agreement with the Florida Department of Agriculture and Consumer Services (FDACS) Division of Administration, for drainage improvements to the midway area at the Clay County Fairgrounds, with the Agreement term ending December 31, 2016, and a grant amount of $100,000.00 (Revenue). Additional approval of Budget Resolution in the amount of $100,000.00 to record unanticipated revenue. Funding Source: 001-001-334715 (Gen Fund Rev -- FDACS Grant-FG) (K. Thomas)

5. Approval to post notice of intent and to award Bid #15/16-9 Resurfacing of Camp Chowenwaw Park Swimming Pool, to Capital T Enterprises, Inc. in the amount of $88,581.00. Approval will be effective after 72 hour bid protest period has expired. A copy of each bid response is attached for review. 6. Approval to post notice of intent and to award Bid #15/16-10, Repairs to Ball Field Lighting at Carl Pugh Park Fields #2 and #4, to Mardant Electrical Construction Co., Inc. Approval will be effective after 72 hour bid protest period has expired. A copy of each bid response is attached for review. 7. Approval to reject the only bid received for Bid #15/16-11, Cameras and Door Security Upgrade at the Emergency Operations Center, as the vendor failed to submit a bid bond as required in the bid. Staff wishes to make minor changes to the scope of work and re-bid this project. A copy of the bid is attached for review. (K. Thomas)

8. Approval of the Continuing Service Agreement with Prosser, Inc., for park design and recreation related projects, for a term of 24 months, at costs to be negotiated per project when assigned a work order for specific tasks, as needed. Funding Source: Various, as needed (K. Thomas)

9. Approval of Military Museum of North Florida Reimbursement Agreement in the amount not to exceed $10,000.00 for reimbursement of operational expenses and facility rehabilitation expenses. Agreement Term: October 1, 2015 –Sept. 30, 2016. Addition approval of funds transfer from General Fund Reserves for Contingency (9900) to cover costs of reimbursement agreement is requested.
